The microbiome has emerged as a key axis in modern oncology, linked to immune responses, immunotherapy efficacy, and tumor progression. The current challenge, however, lies not in identifying correlations but in uncovering the causal mechanisms that underlie them and translating this knowledge into real clinical applications. In this lecture, Sergio Serrano-Villar will discuss the evidence connecting the microbiome with antitumor immunity, ongoing efforts to develop diagnostic biomarkers and microbiome-based therapies, and the methodological pitfalls that risk distorting interpretation in a field as dynamic as it is prone to overstatement.
